The Immunisation Team will be visiting School On Friday 28th November to administer the Flu vaccine to children from Reception up to year 6.
IF you wish for your child to receive the vaccine in school this year, please follow the link below to complete the online Consent Form. If you wish to decline, please use the same link to complete the ‘No Consent’ Form.
